Current Student Forms
Form 1-The Business Petition -Used to request exception to College of Business requirements or rules (not for course equivalencies).
Form 2-Course Equivalent/Substitution Petition - Used to request that a non-FAU course be considered as a substitute or equivalent to an FAU course. This form is not to be submitted prior to enrolling at FAU.
Form 5 -The Reinstatement Petition - Used to request to return to FAU after being academically dismissed from the University.
Form 6 - Application for Upper Division - Used to request admission to College of Business upper-division status, allowing the enrollment in upper-division business courses and the declaration of a major.
Form 7 - Undergraduate Change of Major - Used to declare or change a major in the College of Business.
Form 8 - Undergraduate Major / Minor Change - Office of the Registrar
Form 9 - Credit for Non-Degree Courses - Use this form to apply credits earned as a non-degree seeking student to degree-seeking status.
Form 11 - Residency Petition - Use this form to request a waiver of the requirement to take the last 30 credits of upper-division coursework in residence at FAU.
Form 12 - Acknowledgement of Rule - Used to acknowledge the prohibition of transient course work during the graduating semester. The College of Business policy does not allow students to take transient courses in their graduating semester. This is because the transfer credit cannot usually be processed in time to certify degrees. This form can be used for a student to self-waive this policy, but assume all associated risks. Check with an academic advisor BEFORE submitting this form.
Form 13 - Request to Withdraw from Graduation - Use this form to withdraw a previous request to graduate from the University.
Form 14 - Graduation Audit Request - Used to request a formal graduation audit by an academic advisor. An audit will be performed only if a student has 11 courses or less remaining before graduation (including in-progress classes).
Form 15 - Graduate Student Reinstatement Letter and Petition - To be used by former graduate students who are seeking re-instatement after being academically dismissed from the University.
Form 16 - Graduate Student Readmission Letter and Petition - Used to request readmission to FAU as a graduate student after status becomes inactive.
International Business Certificate Request Form - Used to request a Certificate of International Business (usually filed in your graduating semester).
GRADUATE COLLEGE FORMS
Application for Admission to FAU - Used to apply to FAU's undergraduate degree programs.
Degree Application - Used to apply for your degree (due no later than the 3rd week of your graduating semester).
Enrollment Verification Form - This is a Registrar form, allowing you to obtain documentation from the Registrar's office concerning your enrollment status at FAU.
Forgiveness Policy Form - FAU form that allows students to "forgive" prior grade on a repeated course.
Gordon Rule Evaluation Form - Used by the Registrar's office, to apply transfer work toward the state's Gordon Rule requirements.
Immunization Form - This Student Health form is for the required proof of immunization for all new FAU students.
Transient Student Form - Used for taking courses as a visiting student at other Florida universities and community colleges. This link is to the electronic version, allowing you to submit the form by web, and eliminating the need to hand-carry the form. The electronic form is usually processed in 1-2 working days. Older paper versions are available from the Undergraduate Studies or Registrar's offices, if needed.
